В статье обоснована концепция организационной интеграции стационарного и амбулаторного этапов медицинской реабилитации. Рассмотрены современные модели оказания реабилитационной помощи, выявлены барьеры и возможности межуровневой преемственности. Предложены принципы организационно-функционального объединения двух этапов реабилитации в единую систему сопровождения пациента. Показана значимость смешанных моделей ведения, в которых контроль результатов обеспечивается через телемедицинские технологии, мультидисциплинарную команду и единый цифровой профиль пациента.

The article substantiates the concept of organizational integration of inpatient and outpatient stages of medical rehabilitation. Modern models of rehabilitation care are considered, barriers and opportunities for ensuring continuity between levels are identified. The principles of organizational and functional integration of two stages of rehabilitation into a single system of patient support are proposed. The importance of mixed management models is shown, in which the control of results is provided by telemedicine technologies, a multidisciplinary team and a single digital patient profile.
